Onsite Accompaniment

  • We facilitate onsite health assessments at churches, homeless shelters, transitional housing sites, and other community organizations to meet and engage our neighbors with dignity and respect while understanding and discussing their individual healthcare needs.

    Education is tailored to each neighbor’s individual needs according to the NSON Health Model.

  • Our Nurse Advocates support neighbor whole-person health by providing:

    • Basic nursing assessments (blood pressure, pulse, etc.)

    • Medication reviews

    • Education on health conditions and concerns

    • Resources and referrals based on individual needs

    Nurse Advocates offer compassionate, personalized care that empowers our neighbors to take the next step in their health journey.

    Our non-medical Caring Companions play a vital role in creating a welcoming and supportive environment. They assist with:

    • Signing in patients

    • Engaging in meaningful one-on-one conversations and prayer

    • Helping collect and organize paperwork

    By offering a friendly presence and a listening ear, Caring Companions strive to ensure every neighbor feels seen, heard, and valued.

    Many of our Caring Companions are also Interpreters ensuring that we can communicate with those who seek our support.

1:1 Accompaniment

  • We provide health advocacy, navigation and education on a case by case basis for neighbors seeking additional support and extended accompaniment.

    A team of volunteers personalizes whole-person health by identifying goals and creating a care plan responsive to individual needs according to the NSON Health Model.

    1:1 accompaniment involves multiple meetings (in-person or virtual) over a period of time. 

  • Our Nurse Advocates walk alongside individuals to address for their whole-person health needs. This may entail:

    • Carrying out basic nursing assessments and creating a personalized care plan

    • Connecting neighbors with resources

    • Educating about disease management, prevention, and medications

    • Navigating the healthcare system to obtain necessary care

    • Providing spiritual care and accompaniment

    Nurse Advocates offer compassionate, personalized care that empowers our neighbors to take the next step in their health journey.

    Our non-medical Caring Companions may work with our Nurse Advocates to provide 1:1 spiritual care and accompaniment, as well as overall support of social wellbeing.

    By offering a friendly presence and a listening ear, Caring Companions strive to ensure every neighbor feels seen, heard, and valued.

    Many of our Caring Companions are also Interpreters ensuring that we can communicate with those who seek our support.

Spiritual Accompaniment

  • Spiritual accompaniment and prayer is central to our mission, offering vital support for our volunteers, staff, neighbors, and the entire NSON community.

    Our Prayer Advocacy volunteers regularly pray for the intentions requested by our neighbors during our visits, as well as any prayers requested via our website.

  • Anyone can join our Prayer Advocacy team—whether you live next door, in another state, or overseas. Join us in lifting up our community through the power of prayer, no matter where you are.

The NSON Health Model serves to encourage and support our neighbors in living a well-balanced life and becomes an interactive approach to building a care plan.
